- Parent/Guardian Photo ID
- Insurance Card
- Copay (if required)
- Immunization Record
Important: Please bring your child's current immunization (shot) record to the first visit. Appointments will be rescheduled if an immunization record is not provided.
2Newborn Checklist
- All Newborn Hospital Discharge Papers
- Hospital Immunization (Vaccination) Record
- Contact Your Insurance Plan to Add Your Newborn (Most plans require this within 30 days.)

Vaccines
We follow the American Academy of Pediatrics (AAP) recommended immunization schedule to help keep children healthy and protected.

We prescribe antibiotics only when medically necessary to provide safe, evidence-based care.
Coordinated Care
We coordinate your child's care with specialists and other healthcare providers to ensure the best continuity of care.
Office Appointments
We offer well-child visits and sick appointments. Scheduled appointments are recommended to minimize wait times.
Appointments & Scheduling
Please arrive 15 minutes before your child's well-child (checkup) appointment. Parents and, when appropriate, patients will complete electronic screening questionnaires before seeing the provider. Arriving early helps ensure enough time to complete these forms and keeps appointments running on schedule.
After-Hours Care
A provider is available 24/7 for urgent after-hours concerns. Call our main office for instructions.
Patient Portal & Communication
Securely access your child's medical records, receive appointment reminders, and communicate with our office through our patient portal.
Insurance & Billing
We verify insurance eligibility before your visit. Copays are due at the time of service, and payment plans are available when arranged in advance.
